• Providing the necessary information in line with the requests and audits of regulatory and supervisory institutions and official authorities,
• Maintaining information about your health data that must be kept in accordance with the relevant legislation,

Your personal data processed and collected within this scope; It is mandatory to process personal data belonging to the parties to the contract due to the fact that it is directly related to the establishment or performance of a contract shown in the second paragraph of Article 5 of the Law, data processing is mandatory for the data controller to fulfill its legal obligation, data processing is mandatory for the legitimate interests of the Company, provided that it is made public by the person concerned and does not harm the fundamental rights and freedoms of the person concerned; your personal data of private nature, such as your health data, is processed based on the explicit consent of the person concerned as shown in the second paragraph of Article six of the Law; it is related to the personal data made public by the person concerned and is in accordance with the will of the person concerned to make it public; it is mandatory for the establishment, use or protection of a right; It is necessary for the protection of public health, preventive medicine, medical diagnosis, treatment and care services and for the planning, management and financing of health services by persons under the obligation to keep secrets or authorized institutions and organizations.

If CLINICHAIR will respond to the requests within this scope in writing, it will respond without charge up to ten pages; for each page over ten pages, it will charge a transaction fee of 1 Turkish Lira. If the response to the application is given in a recording medium such as CD, flash memory, the fee that may be requested by the Company shall not exceed the cost of the recording medium.

    In the application that you have as a personal data owner and that you will make in order to exercise your rights mentioned above and that includes your explanations regarding the right you request to exercise; the matter you request must be clear and understandable, the matter you request must be related to your person or if you are acting on behalf of someone else, you will need to submit your special power of attorney certified by a notary public. In accordance with the "Communiqué on the Procedures and Principles of Application to the Data Controller", it is mandatory to include the elements of name-surname, signature, identification number, residence or workplace address, e-mail address, telephone and fax number, and subject of request in your applications.     “I hereby accept and declare that I have read the "Clarification Text on the Protection and Processing of Personal Data of Customers", that I have been informed about the transfer and processing of my personal data within the scope of the Law No. 6698 on the Protection of Personal Data; I have been informed by CLINICHAIR to the relevant recipient groups within the purposes, methods and legal reasons specified in the Clarification Text and that I have been clearly and clearly informed about my rights listed in Article 11 of the Law and how to exercise these rights and that I have been clearly informed that I can withdraw my explicit consent at any time.

    In this context; I approved that provided that it is limited to the principles and purposes specified in the clarification text; to collect, process and store my personal data within the scope of obtaining the necessary permissions recognised in the legislation in order to communicate with me electronically, to make product and service promotions, to inform me about campaigns and promotions, to send me electronic messages with advertising content via e-mail, SMS or call, and to collect, process and store my personal data within the scope of strategic marketing and marketing analysis activities within the scope of my explicit consent and to transfer it to the relevant places specified in the clarification text domestically in accordance with the conditions specified in Articles 8 and 9 of the Law and clearly stated above.
